An Aging Life Care Manager conducts an initial assessment

Allow ASAP’s care management team to complete an all-inclusive assessment which includes but is not limited to:safety, social, financial, and Activities of Daily Living. This will allow our ASAP care management team to have a full grasp on your loved ones situation.Looking at family resources, we recommend options based on your loved one’s preferences and budget. We also give you a general sense of how things are likely to evolve over the next 6 months. This way you can make decisions within the context of a long-term perspective.

An Aging Life Care Manager arranges for in-home care

It may be that your loved one is no longer able to live safely alone.Or, it could be that your loved one requires more constant attention. He or she may not need the services of a nurse but the daily presence of a kind caregiver who will offer basic food preparation,light housekeeping, assist with bathing and grooming, and medication reminders as directed. You can choose part or full time care giving assistance. We make the arrangements so you rest easily knowing your loved one is in good hands with caring and consistent help.

ASAP can provide a National Registered Guardian for crisis situations

ASAP houses one of Huntsville’s, AL first registered Guardian Mrs. Chanda Crutcher CCM. ASAP can assist you and your family with emergency needs. Guided by standards defined by the National Guardianship Association, ASAP can aid you in making important health care decisions based off “Model Code of Ethics for Guardians”.
